If you’re over 65 — or soon will be — and living at home, you’ll want to know about the AT-HM Scheme launching on 1 November 2025. Released by the Department of Health, Disability and Ageing (under the Australian Government), this scheme is part of the broader Support at Home Program.
The good news? This funding is in addition to your regular home-care services. You won’t need to save up from your normal service budget to access it.
What is the AT-HM Scheme?
The AT-HM Scheme is designed to give older Australians access to: Assistive technology: items/equipment that help with daily tasks or mobility (for example, mobility aids, specialised bathroom equipment).
Home modifications: changes in the home environment to improve safety and accessibility (for example, grab rails, non-slip flooring, ramps or redesigning bathroom layout).
If you’ve been thinking, “I can’t reach the bathroom safely anymore” or “I’d like a bench at the right height”, this scheme could be the support you’ve been waiting for.
How does the funding work?
Here are the key details:
The funding is tiered: Low, Medium and High.
For both assistive technology and home modifications, the caps are roughly: under $500 (Low), up to $2,000 (Medium) and up to $15,000 (High).
Most people will have 12 months to use their approved funding from the date of approval. Health professional assessments, prescriptions and training/wrap-around services are covered in full by the government.
Your contribution may apply only to the cost of equipment or modifications. If you’re transitioning from the older “Home Care Packages Program” (HCP), any unspent funds from there may need to be used first before AT-HM funds apply.
What this means for you is that you now have access to dedicated funding for the kinds of products and modifications that help you stay safe and independent at home. You won’t need to delay equipment or home changes because you’re saving up in your usual services budget. You’ll work with an assessor and your provider to identify the best supports for your home and your goals.
You’ll get choice and a say in how the changes are made, with more independence and dignity. Next steps — what you should do
1. If you haven’t done so already, register with My Aged Care and have an aged-care needs assessment. This will determine if you’re eligible for Support at Home and whether AT-HM supports apply.
2. Think about what parts of your home or daily routine are becoming harder — bathroom access, kitchen bench height, mobility around the house.
3. When approved, talk to your care provider about your support plan and ask about the AT-HM Scheme portion.
4. Get quotes, ask for prescriptions (if required), and choose a trusted supplier/installer for assistive tech or home modification.
5. Keep in mind: you typically have 12 months to use the approved funds — SO ACT SOONER RATHER THAN LATER.
